Parents and Guardians of Sixth Grade Students

Dear Parents and Guardians of Sixth Grade Students, 

The Massachusetts Department of Public Health requires that all seventh grade students must show proof of having received one dose of Tdap (tetanus, diphtheria and pertussis) vaccine before attendance is allowed.

To meet this requirement, an immunization record must be provided to the school showing the date your child received the vaccine and a signature or stamp of the doctor or clinic that provided the vaccine. If your child has already received this dose, he or she will not be required to receive it again. 

Tdap vaccine is considered a booster to the DTaP (diphtheria, tetanus and pertussis) vaccine that is required for Kindergarten entry. Tdap vaccine provides continued protection to students from three diseases: tetanus (lockjaw), diphtheria, and pertussis (whooping cough). All of these diseases can have very serious consequences including death. 

Tdap vaccine is available from your doctor, clinic or healthcare provider and is covered by most health insurance plans. Please contact your healthcare provider to schedule an appointment or to find out the hours when vaccines are given. Attached is a copy of the vaccine information sheet.

Two other vaccines are also recommended for all adolescents at 11-12 years of age: meningococcal vaccine (MCV4) and human papillomavirus vaccine (HPV). Although neither of these vaccines are required for school entry, you are encouraged to ask your doctor or healthcare provider about these vaccines when your child receives the Tdap vaccine. 

As the end of the year is quickly approaching, now is the time to schedule your childs physical exam and immunization appointments. As a reminder, any middle school student who wishes play sports must have a physical exam every year or they will not be permitted to play or practice. 

Again, you must provide the school health office with evidence of vaccination prior to the start of your childs seventh grade year. Seventh graders who are not in compliance will not be permitted to attend school per the department of public health.

O Departamento de Sa˙de P˙blica de Massachusetts exige que todos os alunos da sÈtima sÈrie devem mostrar a prova de terem recebido uma dose de vacina Tdap (tÈtano, difteria e coqueluche) antes que a assistÍncia seja permitida.

Para atender a este requisito, um registro de imunizaÁ„o deve ser fornecido ‡ escola mostrando a data em que seu filho recebeu a vacina e uma assinatura ou carimbo do mÈdico ou clÌnica que forneceu a vacina. Se o seu filho j· recebeu esta dose, ele n„o ser· obrigado a recebÍ-lo novamente.

A vacina Tdap È considerada um reforÁo ‡ vacina DTaP (difteria, tÈtano e pertussis) que È necess·ria para a entrada no Jardim de Inf‚ncia. A vacina Tdap oferece proteÁ„o contÌnua aos estudantes de trÍs doenÁas: tÈtano (lockjaw), difteria e tosse convulsa. Todas estas doenÁas podem ter consequÍncias muito graves, incluindo a morte.

A vacina Tdap est· disponÌvel no seu mÈdico, clÌnica ou profissional de sa˙de e est· coberta pela maioria dos planos de seguro de sa˙de. Entre em contato com seu mÈdico para agendar uma consulta ou para descobrir as horas em que as vacinas s„o administradas. Anexado È uma cÛpia da folha de informaÁıes da vacina.

Duas outras vacinas tambÈm s„o recomendadas para todos os adolescentes aos 11-12 anos de idade: a vacina contra o meningococo (MCV4) ea vacina contra o papilomavÌrus humano (HPV). Embora nenhuma destas vacinas seja necess·ria para a entrada na escola, vocÍ È encorajado a perguntar ao seu mÈdico ou profissional de sa˙de sobre essas vacinas quando seu filho recebe a vacina Tdap.

Como o final do ano é rapidamente approching, agora é a hora de agendar o exame físico do seu filho e compromissos de imunização. Como lembrete, qualquer aluno do ensino médio que deseje praticar esportes deve ter um exame físico todos os anos ou eles não serão autorizados a jogar ou praticar.

Mais uma vez, vocÍ deve fornecer ao escritÛrio de sa˙de da escola com a evidÍncia de vacinaÁ„o antes do inÌcio do ano da 7 ™ sÈrie do seu filho. Os alunos do 7∫ ano que n„o estiverem em conformidade n„o ser„o autorizados a frequentar a escola pelo departamento de sa˙de p˙blica.
