We are still waiting for the final Philly trip cost from the tour company. We apologize for the delay. We will let you know as soon as we know about your final balance. Once we know, we will need you to pay it as soon as possible. All funds and forms need to be returned to Eve Heyman. Funds can be paid via check (payable to "Oak Bluffs School Class of 2017"), cash, or you may pay via credit card at the front office, and they will let Eve know you have made your payment.

Complete the following and return them to Eve Heyman by Friday, May 12th:
- completed health form
- copy of the front of your child's health insurance card (this can be done at school if you can't copy it at home) on 8.5"x11" paper.
- permission slip
- memo of understanding
- room list (to be completed with your child)
These forms have also been shared with your child via googledocs.
There will be a trip meeting at the end of May. Families are welcome to attend. We will be in touch soon about the scheduling of the meeting.
